Guidelines:
1. Applicant must be a Madison resident in senior year of high school, planning on further education.
2. Selection will be based primarily on volunteer activities. Please do not include any service hours
required for academic or religious programs.
3. Application must be completed and returned, either by email or snail mail, by March 18th.
4. If application is incomplete, it will be disqualified.
Attach:
1. Essay of no more than 250 words, explaining why you chose to volunteer and what you have
gained from this experience.
2. Recommendation letter(s) from volunteer supervisor(s).
3. High School transcript through and including first semester of Senior year.
